Open image, paint over censor blocks, estimate where lines would go, add tones as necessary. For pixel censor, you basically have to draw whole thing by hand.
Either requires some artistic commitment.
Japanese pornographic laws were revised in Meiji Reformation and were basically ignored after American occupation in WWII while they democratically reformed other laws. They have relaxed somewhat over years (around 1990 all pubic hairs and region had to be censored, recently it is only central public regions). The 1960s and 70s have given rise to other fetish material and homosexuality as international acceptance(?) on the matter influenced Japanese popular culture. Obscenity is controlled by Japanese Criminal law sections 174-176, where 175 deals with printed material.
As you say, some artists do bypass this by selling 'uncensored' versions overseas, but these are almost exclusively digital purchases, as local sales are all printed material and must comply with national law. This is why artists such as Comet release uncensored international version later, and sometimes halt production if piracy is bad.
